Introduction to Medicine

Professor Wilson is a world renowned specialist knee surgeon who has pioneered several techniques in knee ligament and knee osteotomy surgery. He has multiple patents and is a key opinion leader for a number of industry partners.

Adrian qualified with a BSc in Biochemistry from Manchester University in 1989 and then went on to study medicine at St Bartholomew’s medical school, graduating in 1994. He completed his fellowship in Sports Medicine at the Brisbane Orthopaedic Centre with Peter Myers in 2004. Adrian was then appointed as an NHS consultant at the North Hampshire NHS trust which in which he went on to set up as a centre of excellence for knee ligament and osteotomy surgery.

Adrian pioneered the “All Inside” ACL and PCL techniques, performing the first live demonstration of an all inside ACL at the German Arthroscopic meeting in Garmisch in 2011. He was the first to publish on both the all inside ACL and PCL techniques.

Wilson developed and has patented with Arthrex; the concept of using an internal brace with a ligament reconstruction. Adrian first published in this in 2013. He went on to travel the world for a decade teaching about these techniques and paving the way for all inside internal brace to become the gold standard.

ACL Repair

Adrian developed many of the tools involved in the all inside technique and patented the Arthrex graft tube system which Adrian invented and developed with Arthrex. This simple idea is now a standard part of most ACL surgeons daily routine.

Adrian then went on to help with the development of ACL repair and was the first to publish the use of this innovative technique in children. He uses this technique on a routine basis getting his patients back to full activity in 3-4 months as opposed to 9-12 months with the traditional reconstruction techniques.

Adrian pioneered the Anterolateral ligament technique using a free graft and was the first to demonstrate this technique at numerous international meetings. He has published extensively on this topic and is a member of the Anterolateral Ligament Expert group.

On the back of his pioneering work in knee ligament surgery Adrian went onto publish numerous papers and book chapters. He has supervised several PhDs in knee ligament surgery and published in his patented idea of a Tri graft or ‘Trilink’ which was written up as part of Mr Breck Lord’s PhD.

On the back of his pioneering work in the field of knee ligament surgery, Wilson set up The North Hampshire hospital as a tertiary referral centre for complex knee ligament injuries. He set up a regular visitation programme for Consultant surgeons to attend and learn about his pioneering techniques and also set up a fellowship programme and train numerous young Consultants as the final stage in the orthopaedic training.

Knee Osteotomy

Adrian also developed a particular interest in knee osteotomy surgery. He was a co-founder of the UK Knee Osteotomy Registry (UKKOR), a dedicated national database for osteotomy surgery. He went on to set up his Basingstoke Osteotomy Masterclass in 2007 with Neil Thomas, then later brought this course to London in 2016 when he relocated his practice. The Osteotomy Masterclass has hosted the leading osteotomy key opinion leaders who have come to the UK and helped with the teaching and training of knee surgeons in the art of osteotomy surgery. The course moved to London in 2016 and now has international recognition. The London Osteotomy Course now moves around the globe and has been responsible for the training of thousands of surgeons. Adrian and his group are regularly invited to teach at international courses which they do on a monthly basis. The team also now teach in Nantes at the Newclip courses hosted by Skylab. These hands on cadaveric courses have proved to be very popular with surgeons interested in learning about knee osteotomy surgery. Adrian’s group have given over a thousand presentations on the topics of knee preservation and ligament surgery.

Adrian has developed several instruments for osteotomy surgery including the plates used to fix the osteotomies. In particular Adrian Wilson and Kristian Kley pioneered the concept of the Hinge screw which has now been adopted as standard practice for knee osteotomy surgery. Adrian and Kristian have gone on to pioneer minimally invasive techniques for High tibial, Distal femoral and rotational Osteotomy surgery. Adrian has developed and patented several fixation devices and techniques with Newclip Technics, the leading company in knee osteotomy surgery. Adrian together with the other members of his group have gone on to form the Newclip Scientific Advisory board.

Adrian co-founded UKKOR, the UK Knee Osteotomy Registry, which he used to monitor his work and publish papers on knee osteotomy surgery. Adrian was a Vice Chairman of the ESSKA osteotomy committee in 2017 and set up a recognised centre of excellence in knee osteotomy surgery first in Basingstoke and now in London with the London Osteotomy Centre. Adrian helped to launch the ESSKA Osteotomy travelling fellowship. More recently he has brought the world’s leading osteotomy surgeons to London in the form of a dedicated centre namely “The London Osteotomy Centre” and now does joint cases with Ronald VanHeerwaarrden and Kristian Kley for complex knee deformity.

Professor Wilson has strongly supported ESSKA, the key organisation for sports knee surgery in Europe, and one of the largest organisations in the world for teaching and training surgeons. Adrian has been a member of the ESSKA Osteotomy Committee and the ESSKA Arthroscopy Committee. He is frequently asked to give instructional course lectures at the ESSKA bi-annual meetings. He won the ‘Best Paper’ and ‘Best Training Video’ awards at the ESSKA meeting in Amsterdam in 2014.

Professor Wilson set up a successful fellowship training programme for ligament and osteotomy surgery in 2012.

He has a chair at the Manchester Metropolitan University where he is a visiting Professor to the department of Sport & Exercise. He frequently has international and UK visitors to observe both osteotomy and ligament surgery.

In 2018 Adrian co-founded the Regenerative Clinic. This is the largest clinic of its kind in Europe, treating patients with Orthobiologic injections. This delivers alternative treatments for patients with joint issues such as knee arthritis. Since its inception, the clinic has delivered over 3000 micro-fragmented fat injections. Adrian is considered to be an authority in the field of Orthobiologics and is frequently asked to speak for Lipogems who he is a KOL for.

Finally, in January 2024, Adrian co-founded Oveli™ Healthcare with the mission of delivering highly personalised healthcare solutions. By placing patient care at the heart of its operations, Oveli™ Healthcare aims to revolutionise the way healthcare services are provided, ensuring that each patient’s unique needs are met with the utmost attention and care.

Patents

  • Surgical fixation systems and methods:
    A surgical fixation system can include a fixation device, a loop connected to the fixation device, a graft carried by the loop, and a reinforcement material. Surgical fixation systems can be used in various tissue reconstruction procedures, including but not limited to, ACL and PCL reconstructions. The graft and the reinforcement material may be tensioned independently of one another to avoid stress shielding the graft.

  • Hybrid double bundle ACL/PCL graft construct with a single graft:
    Techniques and reconstruction systems for fixation of bone to bone, or soft tissue to bone, that allow a double-bundle type construct with only three tunnels, a single graft and individual tensioning in each of the three tunnels. Alternative double-bundle ACL/PCL fixation techniques are provided that employ a double-bundle type construct with only a single graft and three tunnels/sockets. The double-bundle ACL and PCL techniques and constructs allow individual tensioning of each of the bundles (e.g., tensioning of each of the anteromedial (AM) and posterolateral (PL) bundles of the ACL) and eliminate the need for additional fixation devices (for example, interference screws) to secure the bundles within the bone tunnels/sockets.

  • Graft compression and sizing tubes:
    Graft compression and sizing tubes, methods of graft sizing and compression using such graft tubes, and kits of graft tubes are disclosed. A graft tube aids in graft preparation for a surgery by quickly and consistently compressing a graft to a desired diameter and measuring the length of the graft.

  • Arthroscopy method and device:
    Methods and devices for obtaining an image of a target area of a joint in a subject are disclosed. Such methods and devices may involve positioning at least a part of a device in an aperture in a bone involved in the joint such that the image capture component is in operable relation to the target area. The methods and devices disclosed herein may also be used for and in combination with ligament repair. Methods of surgery utilising this method and devices suitable for use with this method are also disclosed.

Techniques

  • Ligament reinforcement with Internal Brace:
    In 2013 Adrian presented a technique to augment a cruciate reconstruction with a composite polymer fibre tape, which has since become known as an ‘internal brace’.
  • Anterior cruciate ligament (ACL) repair:
    Adrian has used fibre tape extensively for ACL Repair, and it has become especially useful in children’s ACL surgery. His team’s paper, published in 2016, on ‘Paediatric ACL repair reinforced with temporary internal bracing’ is the first to present ACL repair in children. Click here to learn more.
  • All-Inside technique for anterior cruciate ligament (ACL) and posterior cruciate ligament (PCL) repair:
    Adrian has helped to develop a new surgical technique known as the All-Inside ‘translateral’ approach, along with the instruments needed to carry out this pioneering procedure.

  • Anterolateral ligament:
    Adrian has been involved for some years in the Anterolateral Ligament Expert Group, working with Dr Steven Claes and other experts to determine the function and significance of this structure, as well as surgical techniques for its reconstruction.

  • The CoolCut CaliBrator:
    Adrian worked with the manufacturer, Arthrex, to develop the CaliBrator for anterolateral ligament (ACL) reconstruction.
  • Graft compression tubes:
    This was an innovation of Adrian’s that allows the surgeon to downsize any graft – sometimes significantly – in order to take less bone from the patient when drilling the tunnel for anterior cruciate ligament (ACL) reconstruction.

  • Minimally invasive techniques in knee realignment (osteotomy) surgery:
    Adrian has developed a very precise technique both to measure and carry out minimally-invasive knee realignment (osteotomy) surgery using tiny incisions. He has contributed to the development of new plates which are smaller and allow quicker recovery, as well as the use of a bone wedge and new cryotherapy to enhance recovery. The techniques mean that patients can bear their own weight soon after surgery and have a faster and more successful rehabilitation period.
